Position Overview:
The objective of this role is to assess the medical eligibility of individuals applying for the Arizona Long Term Care Services (ALTCS) program. It’s a unique opportunity to help build out processes that will directly impact the lives of patients across AZ and work for the team dedicated to bringing affordable long-term care to as many people as possible.
The ideal candidate will have:
In-depth knowledge of ALTCS medical eligibility criteria
Experience with ALTCS PAS assessments
Understanding of Medicaid/ALTCS rules and clinical documentation
Requirements:
3 years of experience providing social service case management that includes individual planning and delivery of health care services; OR Graduation from an accredited school of nursing, with an active Arizona RN license; OR a Bachelors or Master’s degree with a major in Social Work, Rehabilitation, Counseling, Education, Sociology, Psychology, Nursing or other closely related field and 2 years of experience providing social service case management.
2+ years of experience working with elderly population
Has a patient centered approach to patient education
Direct Experience with ALTCS Eligibility (PAS) assessments
