The Manager, Connected Care Care Program for the Interdisciplinary team is responsible for providing direct supervision of the overall daily and long term operations and performance of a diverse team of healthcare professionals, including pharmacists, social workers, dietitians, and psychologists. You will drive team performance in delivering high-quality, patient-centered care and case management services, addressing the medical, psychosocial, nutritional, and mental health needs of patients in a virtual environment.
This role involves day to day operational oversight, resource allocation, and ensuring compliance with organizational policies and regulations., reporting directly to the Clinical Director, Care & Case Management and Utilization Management.
Job Description:
Leadership and Oversight:
Provide overall leadership and operational direction for the interdisciplinary team.
Ensure implementation of strategic and process changes designed to achieve outlined goals/objectives.
Oversee development of department specific goals and objectives, ensuring alignment with Care Management strategies and Included Health’s vision, mission, and values.
Monitor team performance and outcomes, ensuring alignment with OKRs and organizational goals.
Operational Management:
Oversee day-to-day operations of the team, ensuring efficient workflows and equitable distribution of caseloads.
Quality Assurance and Compliance:
Ensure that all team members adhere to professional standards, ethical guidelines, and regulatory requirements
Monitor and evaluate the quality of performance by the team, implementing improvements as needed.
Staff Development:
Provide leadership, mentorship, coaching, and professional development opportunities for team members.
Communication and Reporting:
Serve as the primary point of contact between the interdisciplinary team and senior leadership.
Prepare and present reports on team performance, patient outcomes, and operational metrics.
Requirements:
- Qualifications:
- Pharmacist
- At least 3 years of people management leadership experience in a healthcare setting.
- Excellent organizational skills and attention to detail required
- Strong interpersonal skills with ability to build relationships with employees
- Data-driven and comfort in using tools to evaluate and optimize the standardization of care across our growing team
- Ability to be agile and balance multiple priorities, while maintaining positive and professional attitude
- Demonstrates professional, appropriate, effective, and tactful communication skills, including written, verbal and nonverbal
Physical/Cognitive Requirements:
- Prompt and regular attendance at assigned work location.
- Capability to remain seated in a stationary position for prolonged periods.
- Eye-hand coordination and manual dexterity to operate keyboard, computer and other office-related equipment.
- No heavy lifting is expected, though occasional exertion of about 20 lbs of force (e.g., lifting a computer \/ laptop) may be required.
- Capability to work with leadership, employees, and members in an appropriate manner.
- Occasional overnight business travel.
$138,380 - $257,388 a year
Zone A: $138,380 - $214,490 + equity + benefits
Zone C: $166,056 - $257,388 + equity + benefits
This range reflects the minimum and maximum target for new hire salaries for candidates based on their respective Zone. Below is additional information on Included Health’s commitment to maintaining transparent and equitable compensation practices across our distinct geographic zones.
Starting base salary for the successful candidate will depend on several job-related factors, unique to each candidate, which may include, but not limited to, education; training; skill set; years and depth of experience; certifications and licensure; business needs; internal peer equity; organizational considerations; and alignment with geographic and market data. Compensation structures and ranges are tailored to each zone’s unique market conditions to ensure that all employees receive fair and competitive compensation based on their roles and locations. In addition to earning a base salary, this role is eligible for a performance-based bonus. Details of the Annual Bonus Plan, including performance metrics, target incentives, and potential earnings, will be discussed during the interview process.
