The Virtual Clinical Scribe – Pre-Charting Specialist is responsible for supporting healthcare providers by preparing clinical documentation prior to patient encounters. This role involves reviewing medical histories, previous notes, diagnostic results, and other relevant data to create accurate, structured pre-chart notes within the electronic health record (EHR). The ideal candidate is detail-oriented, knowledgeable in medical terminology, and capable of working independently in a fast paced virtual environment.
About the Role:
- Conduct thorough reviews of patient charts, including histories, previous visits, labs, imaging, referrals, and medications.
- Pre-chart for scheduled patient appointments by entering structured notes, HPI templates, and clinical context into the EHR ahead of provider review.
- Identify gaps in care, relevant past medical information, and support providers in care planning through complete chart preparation.
- Collaborate virtually with providers to ensure documentation accuracy and alignment with clinical workflow.
- Maintain confidentiality and comply with HIPAA regulations at all times.
- Support documentation consistency across encounters by following provider-specific preferences and organizational templates.
- Assist with updating problem lists, medication reconciliation, and preventive care reminders when appropriate.
- Communicate with clinical teams as needed to clarify or escalate missing documentation or issues in the medical record.
About You:
Education:
High school diploma or GED required.
Associate’s or Bachelor’s degree in a health-related field preferred.
Experience:
At least 1 year of experience as a medical scribe, clinical documentation assistant, or EHR support specialist.
Prior experience in cardiology is strongly preferred but not required.
Prior experience with pre-charting or working in a virtual clinical support role strongly preferred.
Skills and Competencies:
Proficient in medical terminology, medications, anatomy, and common disease processes.
Strong attention to detail and organizational skills.
Excellent written communication and data entry accuracy.
Ability to work independently with minimal supervision.
Comfortable using video conferencing, messaging platforms, and remote work tools.
Reliable internet connection and secure home office environment.
Work Environment
- Fully remote position; must have access to a quiet, private HPIAA compliant workspace and stable internet.
- Must be available during clinical hours for assigned providers.
- Compliance with data privacy and security standards is mandatory.
