Overview
Chronic Care Staffing is seeking a compassionate and detail-oriented Remote RPM Coordinator – CMA to support patients living with chronic or high-risk conditions through Remote Patient Monitoring (RPM). In this role, you will monitor biometric data, educate patients on device usage, escalate critical findings, and act as a crucial liaison between patients and their healthcare providers. Your work helps improve patient outcomes, promote adherence to care plans, and reduce hospitalizations through real-time monitoring and timely intervention — all from a fully remote, HIPAA-compliant home office.
Responsibilities
Conduct patient onboarding and device education calls for new RPM enrollees.
Monitor daily biometric data (blood pressure, glucose, weight, oxygen saturation, etc.).
Identify abnormal readings and follow escalation protocols to notify providers.
Perform monthly RPM engagement calls and address patient concerns.
Troubleshoot device connectivity or usage issues.
Communicate with patients and caregivers about their conditions, goals, and progress.
Support patient self-management and adherence to provider-developed care plans.
Coordinate with healthcare teams to ensure timely responses to alerts.
Document all interactions and interventions accurately in the EHR/RPM portal.
Educate patients on lifestyle modifications, risk reduction, and monitoring importance.
Comply with CMS RPM billing and documentation requirements.
Meet productivity expectations for monitoring and outreach.
Represent Chronic Care Staffing professionally and compassionately at all times.
Unencumbered active CMA or RMA certification.
Active BLS certification.
Ability to communicate clearly and compassionately via phone or messaging.
Excellent documentation, time management, and critical thinking skills.
Experience with EHR systems and cloud-based platforms.
Familiarity with Google Suite.
Nice to Have (Optional)
- Strong understanding of Remote Patient Monitoring protocols and platforms.
- Proficiency in recognizing clinical red flags in biometric data.
Home Office Requirements
- A HIPAA-compliant workspace free from distractions.

- Use of a CCS-approved computer with dual monitors.
Physical Demands
