Clinical Care Specialist - Registered Dietitian

As a Clinical Care Specialist at 9amHealth, you’ll provide personalized, virtual care and education to patients managing chronic cardiometabolic conditions, working closely with our physicians and care team.
This per diem, W2 role offers the flexibility to support patient care on a part-time basis as needed, with scheduling based on mutual availability.
What you’ll do
Deliver expert care, education, and lifestyle support for patients managing diabetes, hypertension, hyperlipidemia, and weight loss through chat, phone, and video consultations.
Develop consult notes that align with internal protocols and support physician treatment plans.
Partner with Patient Care Advocates to coordinate care and ensure successful outcomes.
Design personalized, evidence-based nutrition and lifestyle plans aligned with each patient’s medical conditions, preferences, and goals.
Collaborate with our pharmacist team on medication-related questions, ensuring comprehensive patient support.
Monitor patient progress through follow-ups, adjusting recommendations based on outcomes and feedback.
Apply motivational interviewing and coaching techniques to encourage sustainable behavioral changes in nutrition, activity, and stress management.
Help patients interpret health data and lab results to promote informed decision-making.
Maintain accurate documentation of care provided in accordance with internal policies and legal standards.
What you’ll need
Minimum Qualifications
Registered Dietitian Nutritionist with active licensure.
Certified Diabetes Care and Education Specialist (CDCES), Board Certified–Advanced Diabetes Management (BC-ADM), or eligibility to obtain certification.
At least 5 years of clinical experience in managing chronic conditions, including diabetes, hypertension, hyperlipidemia, and obesity.
Proficiency in medication management and comfort working within an EMR system.
Demonstrated experience in telehealth or virtual care delivery.
A quiet, professional home workspace and reliable high-speed internet connection.
Strong communication skills and a patient-centered, empathetic approach to care.
Preferred Qualifications
Experience at a digital health company or with remote care models.
Multilingual, with Spanish language fluency preferred.
