Job description
A Behavioral Health Patient Care Coordinator plays a key role in guiding patients through the mental health care journey.
What You’ll Do:
- Conduct telephonic or video-based intake, gather relevant patient information, and identify immediate needs or concerns.
- Coordinate and manage patient appointments, following established guidelines.
- Maintain accurate records in EMR/EHR
- Direct patients to appropriate remote services or support groups
- Address concerns with empathy and professionalism, contributing to a positive patient experience
- Knowledge of medical terminology
What We’re Looking For:
A minimum of 1 year of experience as a bilingual Medical VA.
Proficient in Spanish and English.
Proficiency in medical software and EMR/EHR systems, cClinicalWorks preferred.
Experience in behavioral healthcare.
Excellent attention to detail and accuracy
Excellent communications skills
Knowledge of medical terminology.
