About the Role:
MEDVA is seeking a highly motivated and compassionate Remote Diversion Specialist with a background in healthcare or social services to provide critical support to individuals experiencing housing crises. This remote role extends beyond initial triage, encompassing proactive identification of housing resources, assistance with complex housing applications, and direct client advocacy to ensure access to necessary services.
This is a unique opportunity for qualified Medical Doctors (MD), Registered Nurses (RN), and similarly trained medical professionals to apply their expertise in a meaningful and flexible remote capacity.
Key Responsibilities:
- Conduct comprehensive triage of urgent client situations to accurately assess immediate needs related to housing, safety, and health.
- Coordinate move-in logistics and short-term emergency assistance.
- Proactively scout and identify suitable housing options, including transitional, temporary, and permanent units, based on client needs and availability.
- Provide comprehensive support to clients by completing and submitting complex housing applications accurately and efficiently.
- Coordinate essential move-in logistics and facilitate access to short-term emergency assistance as required.
- Serve as a dedicated advocate for clients, effectively communicating with housing agencies, landlords, and service providers to ensure timely access to resources and equitable treatment.
- Facilitate seamless referrals to crucial care coordination, mental health services, medical care, and relevant community support programs.
- Maintain meticulous and up-to-date case documentation, including detailed tracking of referrals, interventions, and client outcomes, adhering to established protocols.
- Collaborate effectively with U.S.-based teams, ensuring seamless communication and adherence to all HIPAA regulations and confidentiality protocols.
Ideal Candidate Profile:
Essential:
- Native or near-native fluency in both English and Spanish (written and spoken) is mandatory to effectively communicate with diverse client populations and stakeholders.
- Background as a Medical Doctor (MD), Registered Nurse (RN), or other relevant healthcare professional is a fundamental requirement for this role.
- Must possess a reliable personal computer with a Windows operating system and a stable, high-speed internet connection. (Please note: macOS computers are not compatible with our current systems.)
- Demonstrated strong written and spoken English language skills; proficiency in Spanish is a critical requirement.
- Highly organized and detail-oriented with proven experience accurately completing government or service-based applications.
- Proficient in utilizing various online platforms and case management CRM systems.
- Exceptional communication and interpersonal skills, coupled with a strong commitment to client advocacy.
- Comprehensive understanding of the complexities of housing instability, the principles of trauma-informed care, and the impact of social determinants of health.
- Availability to work U.S. business hours and US Time Zones.
Preferred but Not Required:
Familiarity with U.S. housing systems and resources, including Coordinated Entry, Section 8, and Rapid Rehousing programs.
Knowledge of wraparound services, such as mental health support, substance use resources, and benefits navigation assistance.
Previous experience in case management or care coordination roles.
