Work Arrangement: Fully remote, Monday to Friday, with flexibility
Job Type: Full-time, Independent Contractor.
Work Schedule: 40 hours per week, ideally with consistent morning coverage (EST/PST preferred)
Locations: Remote, open to candidates worldwide with availability during Eastern Time or Pacific Time hours.

About the Company:
The Company is a virtual-first specialty clinic focused on delivering compassionate, evidence-based care to patients with rare and complex chronic conditions including Ehlers-Danlos Syndrome (EDS), Postural Orthostatic Tachycardia Syndrome (POTS), Mast Cell Activation Syndrome (MCAS), and related disorders. They break down systemic barriers to access and provide care through cutting-edge digital health tools. Its team includes top-tier academic backgrounds (Stanford PhD, Notre Dame Pre-Med) and is dedicated to the patient experience from first contact through ongoing treatment.
Role Overview:
As a Care Operations Coordinator, you’ll be the frontline connection between patients, providers, and the broader care team—helping ensure a smooth, supportive, and well-coordinated care journey. You’ll handle everything from scheduling and prescription coordination to navigating referrals, prior authorizations, and follow-ups. This position blends patient-facing communication, cross-functional coordination, and light operational support, with a focus on using automation where possible while maintaining a human touch where it matters most.
Key Responsibilities:
Patient Support & Communication (50%)
- Serve as the first point of contact for patient inquiries via EHR, secure messaging, and phone.
- Manage scheduling, prescription coordination, and follow-ups with clarity, empathy, and professionalism.
- Respond to inbound messages and provide timely support to patient concerns.
Care Coordination & Navigation (40%)
- Collaborate with internal clinical teams and external providers to coordinate diagnostic testing, imaging, referrals, and prior authorizations.
- Track and document patient progress throughout their care journey, ensuring a seamless and continuous care experience.
Operational Workflow Support (10%)
- Maintain accurate, HIPAA-compliant documentation.
- Support new patient onboarding and proactively flag care gaps, as well as systemic or financial barriers to care.
- Participate in team meetings and contribute to continuous process improvement initiatives.
- Use automation tools to streamline workflows, while handling essential manual tasks like chart preparation and patient communication.
1st Week Immediate Tasks:
- Shadow and learn how we document in the EHR (Elation Health) and handle patient messages
- Start replying to simple patient inquiries, with guidance
- Help with new patient intake and onboarding
- Join our weekly ops meetings and start contributing
- Get trained on our care model and key conditions we treat (EDS, MCAS, POTS)
Must-Haves
- 1–2 years of experience in U.S. healthcare (clinical, operational, or support roles)
- Prior experience in care coordination, case management, or virtual clinical support
- Familiarity with U.S. healthcare systems, including insurance, referrals, and prior authorizations
- Strong operational skills and attention to detail
- Ability to understand care visit processes and make basic clinical judgment calls
Nice-to-Haves
- Experience using Elation EHR or similar digital health platforms
- Background working in VC-backed digital health startups
- Exposure to chronic or complex care management (e.g., EDS, MCAS, POTS)
- Comfort navigating fast-paced, remote team environments
