9amHealth is specialized cardiometabolic care —a first-of-its-kind, whole-body approach to preventing and treating obesity, diabetes, high blood pressure, and high cholesterol, and help with weight management. 9amHealth was founded in 2021 by the team behind mySugr (acquired by Roche in 2017) and has brought together a team of leaders from across cardiometabolic healthcare along with the backing of leading investors like Cigna Ventures, 7Wire Ventures, and Founders Fund.

What you’ll do
Provide comprehensive diabetes, hypertension, hyperlipidemia and weight loss coaching, education, and support for patients through chat, phone, and video calls following internal protocols
Create thorough consults aligned with internal protocols for 9amHealth physicians
Collaborate with RDs, PharmDs, and Patient Care Advocates to ensure patient success
Support members in executing nutrition and lifestyle plans created by 9amHealth RDs for patients based on their medical conditions, goals, and preferences, ensuring they align with evidence-based guidelines for managing diabetes, hypertension, hyperlipidemia, and obesity
Track patient progress through regular check-ins, assess the effectiveness of interventions and refer members to RDs, PharmDs, and providers to adjust care plans as needed based on clinical outcomes and patient feedback.
Provide motivational interviewing, coaching, and counseling to support sustainable behavior changes in patients regarding diet, exercise, and stress management.
Utilize patient data and health metrics to inform coaching recommendations, helping patients understand lab results and tracking outcomes like weight, blood pressure, and cholesterol levels.
Maintain accurate and thorough patient documentation, ensuring all care actions, recommendations, and follow-ups are documented according to internal and legal standards.
As a per diem coach, your schedule may vary based on patient demand and business needs.
What you’ll need
Minimum Qualifications
An active health coaching or related certification (e.g., NBHWC, Wellcoaches, Duke Integrative, DPP Lifestyle Coach, CDCES, or BC-ADM).
Deep understanding of cardiometabolic conditions, especially diabetes, hypertension, hyperlipidemia, and obesity.
At least 5 years of experience in direct patient care—ideally in clinical or virtual environments—with a focus on chronic condition support.
Familiarity with EMRs and virtual health platforms.
Strong communication skills and the ability to foster patient trust and engagement through empathy and personalized support.
Experience working within interdisciplinary care teams alongside clinicians, dietitians, and support staff.
Comfort delivering care virtually across chat, video, and phone using modern digital tools.
A secure, distraction-free home workspace that meets privacy and confidentiality standards.
A stable high-speed internet connection capable of supporting uninterrupted video and messaging sessions.
Preferred Qualifications
Prior work at a digital health company focused on chronic conditions.
Multilingual capabilities, particularly in Spanish, to support diverse patient populations.
