Summary
Join RPM Healthcare as a Care Coach for Chronic Care Management (CCM) and work remotely with a State license. As a licensed LPN or LVN, you will develop personalized CCM care plans, engage patients, educate them through shared information, assess and provide resources for Social Determinants of Health, and attend virtual meetings.
Requirements
- Possesses a Nursing License in NJ and or NY
- Knowledgeable about the basics of chronic health conditions
- Proficient with technology
- Comfortable navigating computer platforms and familiar with smartphone functionality
- Comfortable with telephone outreach and engagement
- Excellent communicator (written and verbal)
- Highly organized and efficient
- Detail-oriented and good at multi-tasking
- Team Player
- Exhibits a strong sense of ambition, self-motivation and self-discipline
Responsibilities
- Develop a personalized CCM care plan for each assigned patient
- Serve as the point of contact for patients enrolled in the RPM365 platform by their healthcare professional
- Engage regularly with all assigned patients to encourage consistency with the treatment plan
- Schedule regular monthly check-ins with the patients
- Educate patients through shared information (articles, news, etc.)
- Assess and provide resources for Social Determinants of Health (SDoH)
- Attend virtual meetings with RPM Healthcare's team for information updates
