The Role
Synapticure is seeking a Director of Implementation to lead the successful onboarding and rollout of strategic partnerships with ACOs, health systems, health plans, and pharmaceutical companies. This role is central to our growth and mission to serve patients and caregivers nationwide. Reporting to the Head of Growth, you will be responsible for managing multiple complex implementations simultaneously, aligning cross-functional teams, and ensuring every launch is executed with excellence, speed, and long-term success in mind.
You will oversee workstreams across clinical, product, marketing, data, and operations to bring new partnerships to life while creating scalable, repeatable processes. The ideal candidate is a proven project leader with exceptional organizational skills, deep healthcare knowledge, and a history of delivering results through structured process, stakeholder management, and cross-functional collaboration.
Job Duties – What you’ll be doing
- Lead the end-to-end implementation process for new strategic partnerships, ensuring timely, successful launches across multiple concurrent projects
Program Management Office (PMO) Leadership

- Coordinate cross-functional workstreams, including clinical workflow design, data integration, technical setup, patient and practice rollout, and marketing initiatives
- Build and manage detailed implementation plans, tracking milestones, owners, and dependencies across all phases of the launch
- Surface and escalate risks proactively; drive structured problem-solving to address issues in real time
- Manage detailed program dashboards to report out on implementation progress and performance metrics to executive stakeholders, internally and externally
Process and Documentation Standardization
- Develop and continuously refine implementation playbooks, templates, and tools to support efficient and scalable partner onboarding
- Recommend and implement internal improvements that reduce friction, clarify accountability, and improve cross-team collaboration
- Document key learnings, partner needs, and best practices to inform future implementations and continuous improvement efforts
- Drive consistency across all implementation processes to ensure quality and replicability while accommodating partner-specific needs
Partner Relationship Management
- Serve as the implementation point of contact for new partners, ensuring a smooth and professional experience from kickoff through go-live
- Build trust and credibility with partner stakeholders by clearly communicating plans, timelines, and responsibilities
- Collaborate with clinical, operational, and IT leaders at partner organizations to align workflows, solve challenges, and support adoption
- Translate partner feedback into process or product improvements and ensure a strong handoff to long-term account owners post-launch.
Requirements – What we look for in you
- 5+ years of experience in healthcare implementation, customer success, or project management, with at least 3 years in a healthcare or health tech environment
- Proven ability to manage complex, cross-functional implementation projects with multiple stakeholders and tight timelines
- Strong understanding of healthcare delivery models, clinical operations, and data systems such as EHRs, claims systems, or HIEs
- Highly organized with excellent attention to detail and the ability to manage multiple concurrent workstreams
- Results-oriented, proactive problem solver with a strong sense of ownership and accountability
- Exceptional interpersonal and communication skills, with experience influencing cross-functional teams
- Proficiency with project management tools and techniques (e.g., Asana, Smartsheet, dashboards, workflow diagrams, gantt charts, risk registers)
- Experience in value-based care, health system partnerships, or ACO environments strongly preferred

$135,000 - $150,000 a year

This is a remote-first role. Occasional travel (up to 10%) may be required for partner meetings, team gatherings, or implementation site visits.
