About the Opportunity
Pair Team is hiring a full-time Registered Nurse (RN) to support our expansion into Chronic Care Management (CCM). This role is a core clinical function within our CCM program, focused on proactive, longitudinal care for adults with multiple chronic conditions.
You will partner closely with a Nurse Practitioner and Community Health Worker to provide continuous clinical support for members enrolled in Chronic Care Management. This role centers on proactive outreach, clinical follow-up, and care coordination for adults managing chronic conditions, allowing you to build longitudinal relationships while supporting members between provider visits.
This is a fully remote position, operating on Pacific Time business hours, with a clearly defined caseload and structured engagement expectations.
What You’ll Do
- Manage a defined caseload of enrolled CCM members
- Complete required CCM minimum engagement activities and documentation
- Conduct proactive outreach based on NP notes and care plan needs
- Perform clinical triage for symptom changes, medication questions, and emerging medical concerns
- Provide chronic condition education and ongoing clinical support
- Coordinate with primary care providers and specialists to support continuity of care
- Escalate clinical concerns to the Nurse Practitioner as appropriate
- Develop, update, and document individualized care plans
- Collaborate closely with the NP and CHW to deliver team-based care
- Maintain accurate, timely documentation to support CCM compliance
What You’ll Need
- Active California Registered Nurse (RN) license (required)
- Bilingual - Spanish and English fluency required
- Minimum of 1 year of RN experience in care management or case management
- At least 1 year of experience working with a Medicare population
- Experience supporting patients with chronic conditions (e.g., diabetes, hypertension, COPD, heart failure)
- Strong clinical assessment, triage, and documentation skills
- Comfort working in structured, metrics-driven clinical programs
- Experience coordinating care with PCPs, specialists, and pharmacies
- Ability to work independently in a fully remote environment
- Proficiency with EHRs and virtual communication tools
