The Role
Belle is seeking a Practice Manager to lead and strengthen our Chronic Care Management (CCM) practice operations. This leader will establish and refine processes to ensure compliant, efficient delivery of CCM services—including intake, eligibility verification, patient enrollment, and billing.
Reporting directly to the COO, the Practice Manager will oversee the patient journey, ensuring our work meets Medicare requirements while delivering exceptional patient and provider experiences. The ideal candidate brings small-practice management expertise, thrives in high-growth environments, and is eager to take ownership of building and scaling new processes.
This role is fully remote.
Key Responsibilities
- Design and implement processes to support compliant, efficient delivery of CCM services
- Oversee intake, eligibility verification, enrollment, consent management, billing, and ongoing patient engagement
- Work directly with patients and stakeholders to refine processes, achieving efficiency and compliance
- Monitor CMS requirements and ensure accurate, compliant coding and billing for CCM services
- Maintain data integrity across the CCM workflow, from service delivery through claim submission
- Act as a super user of the RCM solution and maintain accurate dashboards and reports to monitor performance
- Support training and quality standards for staff on CCM services, Medicare billing, coding, and insurance eligibility
- Collaborate closely with the clinical team to support integrated service delivery
- Collaborate with intake and scheduling teams to ensure efficient patient onboarding and service delivery
- Ensure clear, consistent patient communications related to CCM services, EOBs, and expectations
- Lead continuous improvement initiatives to enhance patient outcomes, operational efficiency, and regulatory compliance
You Might Be a Fit if You:
- 3+ years of experience managing operations in a small practice or healthcare setting, specifically with Medicare programs
- Strong knowledge of Medicare billing, coding, and compliance requirements
- Experience optimizing both clinical-adjacent and non-clinical workflows
- Deep knowledge of RCM principles
- Expertise in RCM platforms and medical claims processes (including HCFA 1500)
- Proven ability to design and scale practice workflows in a fast-paced, high-growth organization
- Excellent communication, problem-solving, and organizational skills
- Ability to take ownership, build from scratch, and drive key initiatives
