We provide home-based, foot-forward chronic care management services. We focus on medically complex and hard-to-reach populations, particularly Medicare Advantage members, serving ACO and health plan clients nationwide. Established in 2014, we have 51-200 employees.

Our services include medical pedicures, whole-health assessments, assisted telehealth visits with a virtual clinical team, and creating and maintaining care plans, including referrals to other providers. Preventive care includes foot screenings, light exfoliation, nail care, and soothing massage to boost circulation and skin health.

Our Community Health Workers provide in-home care, supported by an integrated care team that reviews visits. Our clinicians intervene for additional support when necessary, and the integrated care team is available ²⁴⁄₇. Our model aims to reduce the total cost of care, improve quality, and increase member engagement and retention, often generating program ROIs between 2x-4x.

We serve eligible members across Georgia, Texas, Missouri, and Kansas. Data from health plans, involving over 350,000 people, showed that regular visits were associated with a 58% reduction in open wounds, a 52% reduction in falls, a 48% reduction in nursing home needs, a 40% reduction in major depression, and a 14% reduction in ER or hospital visits.

We offer competitive compensation, with stable pay guaranteed weekly, regardless of appointment volume. Employees can earn up to $300 per month in additional benefits.
