Requirements
- RN license in good standing
- Endless empathy for people, and a strong ability to fight for those who can’t
- Strong clinical skills with strong organizational skills to match
- Pride in your technical savvy. You have the ability to quickly and fluently learn new systems and softwares

Responsibilities
- Learn the Solace systems, tools, technology, partners, and expectations
- Build strong, trusting relationships with Medicare patients
- Be able to identify and prioritize Medicare patients' needs and assist them to maintain a streamlined care continuum
- Develop patient care plans to holistically address social determinants of health
- Build the systems of the future in working with Medicare patients
