Evergreen Nephrology

Founded in 2021, we provide value-based kidney care for patients with Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD). We partner with nephrologists and other providers, focusing on the whole person’s physical, emotional, mental, and social well-being, not just the disease itself.

We simplify kidney care by connecting patients with an integrated team that includes nurse care managers, social workers, dietitians, and care coordinators. This team works alongside primary care physicians and nephrologists. Our model supports nephrologists in value-based care with clinical teams and data insights. We help patients navigate the healthcare system, access resources, and get education, removing barriers to care. We also work with payors on prevention to improve quality, member experience, and manage costs. Our expertise covers areas like kidney care management, integrated care, and behavioral health integration. Our team roles range from financial and strategic development to direct patient care and operational support, including positions like Post Acute Nurse Care Managers, Patient Engagement Specialists, Social Workers, Care Coordinators, and Clinical Documentation Improvement Partners.

Our Care team delivers compassionate, whole-person care through roles like nurse care managers, social workers, dietitians, and care coordinators. This team integrates medical knowledge and empathy as they collaborate across specialties. Our Support and Strategy team works behind the scenes, creating systems, tools, and partnerships to expand our care model. This includes professionals in operations, technology, finance, and patient engagement, all dedicated to transforming kidney care. Many of our roles are primarily remote, and we provide the necessary equipment.

We offer full-time employees a flexible time off policy and 12 paid holidays per year. We provide a 401(k) program with an employer match of up to 4% of contributions, which begins after six months of employment. Paid parental leave is also available. Compensation includes competitive base pay and annual bonus eligibility. For remote or hybrid positions, we provide telephony applications and equipment.
