Founded in 2019, we partner with health agencies and organizations to develop digital solutions. We focus on digital health, co-creation, public health, mental health, behavioral health, rapid deployment, and participatory development. Our goal is to improve care delivery and coordination.

Our Chorus platform coordinates whole-person care across behavioral and community health systems. It’s configurable, supports various team workflows, and connects across networks to track and close referral loops. Our solutions include:

• Crisis Dispatch & Response: This streamlines intake, offers ²⁴⁄₇ crisis integration, real-time bed tracking, and automated eligibility checks.
• Closed Loop Referrals: This provides network-wide tracking with two-way data flow to monitor client service connections.
• Longitudinal Care Management: This offers a single view to coordinate client journeys across behavioral health, physical health, housing, and social services.
• Reporting & Billing: This creates audit-ready reports and helps demonstrate outcomes.

We serve Community-Based Behavioral Health organizations, State & County Health Agencies, and Federally Qualified Health Centers. Our platform is for frontline staff, uses flexible templates, and is HIPAA and SOC 2 Type II compliant with encryption. It also integrates via HL7 and FHIR.
