Our easy-to-use mobile app supports activities like taking medication, checking clinical values (e.g., A1c, blood pressure), using medical devices, attending appointments, and getting screenings and labs. It’s available on iOS and Android.

Our approach empowers members with incentives and a support system to build healthy habits. The program has shown strong results: 90% average care plan adherence, a 2-4x ROI on reduced care costs, and a 43% average reduction in high-cost utilization. We’ve also achieved 4+ Star Ratings across Part C & D measures, 16% increased PDC medication adherence, and 51% lower inpatient admissions.

We work with health plans, providers, and other organizations in areas like population health management, ACO, IDN, Medicare, Medicaid, and value-based care. Our technology includes mobile applications, machine learning, and AI. We were founded in 2014.
