Join our team and make a positive impact on the lives of others! We are looking for a Registered Nurse (RN) for our Connecticut team.
This is a hybrid position at our West Hartford, CT office.
Department & Position Overview:
The Team Lead of Patient and Caregiver Services (LPN or RN) will have an innovative, entrepreneurial spirit as well as a passion for continually improving the way we do things. This is a leadership role that will partner with us in our efforts to continually and iteratively make every aspect of our services better for patients, while maintaining appropriate requirements of the Structured Family Care, state, and federal regulations.
This person will be responsible for the traditional parts of a team lead role: planning, organizing, coordinating, evaluating, and providing clinical guidance and supervision of patient care for Freedom Care’s Connecticut Home Care activities. The Team lead will report directly to the General Operations Managers.
Every Day You Will:
Leadership
- Hire, oversee, mentor, and evaluate nursing and contact center teams, focused in patient care.
- Conduct monthly meetings with health coaches to address equipment, scheduling, goals and objectives, performance metrics and improvement activities, and any staff concerns.
- Educate health coaches and care team staff on new and existing department procedures and systems (e.g. information systems, fiscal responsibility, etc).
- Recommendations and provides opportunities for training and development of the team and family caregivers.
Staffing & Operations Management
- Develops strategic staffing plans to ensure adequate staffing coverage in accordance with established productivity measures based on patient population needs.
- Oversees and monitors service/unit operational performance metrics inclusive of satisfaction, baseline agency and overtime costs, staffing and turnover rates.
- Clinical Practice collaboration with health coaches establishing care standards, evaluating overall staff compliance to deliver clinical outcomes.
Regulatory Compliance & Quality Improvement
- Maintains current knowledge and ensures compliance with all regulatory and professional standards of care, as it relates to Medicaid and Structured Family Care; including but not limited to DOH, DOL and CMS.
- Participate in audits, inspections, and surveys from regulatory agencies.
- Utilizes creativity in developing quality/performance improvement programs, instructing Clinical Management, and implementing new and improved standards.
- Embeds quality protocols into standard operating procedures and evaluates them regularly.
Budget Management
- Develops, prepares and monitors the agency personnel, supply and equipment budgets in collaboration with the Care Managers, Personal Care Attendants and supports department directors.
Patient Care & Safety
- Ensures accountability in support services essential to patient care.
- Promotes a safe and caring environment for staff and patients that is conducive to positive health teaching, and maintenance; patient and family expectations of care; develops mechanism to assess same.
- Ensures that the overall planning process occurs for each unit within the service line.
- Analyzes findings from patient satisfaction surveys to establish service-based customer services agenda and ensures that unit-based agendas are developed with input from the nursing management and the staff.
- Ensure completion and accuracy on patient assessments, develop care plans, and ensure services align with patient needs.
- Serve as the primary liaison with health plans and FreedomCare related to patient care and services.
- Investigate and address patient complaints or incidents, ensuring resolution and reporting as required, and supervise timely resolution.
- Cover patient visits as needed to ensure continued care and compliance with agency standards.
Ideal Candidate Will Possess:
- Valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) License, to operate in the state of Connecticut. RN will be required to oversee any other nursing staff.
- Min 2-5 years of experience in a Leadership a role with strong ability to lead and manage a team effectively, strong preference for previous experience on patient care management and/or related home care setting.
- Competency in clinical compliance and documentation, essential for ensuring high-quality patient care and regulatory compliance.
- Effective communication skills (written and verbal), facilitating effective collaboration with teams, patients, and families.
- Strong attention to detail, ensuring accuracy in documentation and adherence to protocols.
- Ability to effectively manage administrative, clinical, and caregiver staff in an office, remote and field setting.
- Highly organized, capable of managing multiple tasks and priorities efficiently.
- Motivated self-starter and creative problem-solver, comfortable navigating and resolving challenges in a fast-paced, dynamic environment.
- Proficient in navigating technology and systems to enhance operational efficiency and effectiveness in healthcare delivery.
- Ability to multitask and work in a fast-paced environment.

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $90,000 and $100,000 per year at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
