Custom Health is a technology-enabled healthcare company that helps people take medications safely and as prescribed. We aim to reduce uncertainty for patients, caregivers, and care teams.

We address situations where people manage multiple medications for chronic or complex conditions without knowing if their therapies are taken consistently or are effective. Unnoticed missed doses, side effects, or health changes can lead to complications and higher costs.

Our approach combines connected medication management, pharmacist-led clinical support, remote patient monitoring, and real-time data, all delivered into the home. This is powered by our proprietary technology, which connects patients, pharmacists, and care teams in real time. Patients use simple digital tools and personalized medication programs to help them take medications on time and share important health information, including vital signs and patient-reported insights. Our licensed pharmacists and clinical staff check in with patients, watch for trends, identify potential issues, and offer support.

We work with prescribing clinicians and healthcare organizations by offering medication management, remote clinical support, and data from connected monitoring tools. The patient’s licensed healthcare provider always makes clinical decisions.

We offer an in-home medication dispenser for easy adherence, Bluetooth-enabled monitoring devices (like blood pressure cuffs, glucose monitors, and weight scales) that share real-time data, and a two-way camera for telehealth visits. We report that 98% of our users achieve medication adherence at home. In 2024, we remotely monitored 7.6 million medication doses. Custom Health operates seven pharmacies and employs over 100 clinical staff, including in-house pharmacists with over 20 advanced certifications who collectively speak more than 20 languages.
