Cloverleaf Care

We are an in-home care agency founded in 2006. We provide medical and non-medical care services for seniors and adults with special needs, helping them maintain their independence and quality of life. Our services include non-medical in-home care, aging-in-place support, and education and advisory services for planning and living at home.

Our trained caregivers offer companionship and personal care. We deliver care in various settings, including clients’ homes, independent and assisted living centers, hospitals, and nursing homes. We offer specialized in-home care for conditions such as Alzheimer’s, dementia, stroke recovery, rehabilitation, telecare, home transitions, hospice care, post-surgery recovery, diabetes, Parkinson’s, and cancer. We also provide support for new mothers, respite care, and short-term recovery care.

We serve areas including San Jose, Santa Clara, Saratoga, Los Gatos, Sunnyvale, Cupertino, Palo Alto, San Mateo, Fremont, Santa Cruz, Gilroy, Monterey, Pleasanton, Castro Valley, Dublin, Brentwood, Lodi, and Stockton. Our administrative team handles scheduling, credentialing, HR support, and coordination for healthcare workers, including caregiver screening and hiring, client shift staffing, staffing report preparation, and payroll and HR activities. Our care services are available ²⁴⁄₇.
