About Illumination Health + Home

We are a nonprofit 501©(3) organization founded in 2008. We provide interdisciplinary services for homeless adults and children across Southern California, working to end homelessness. We help individuals and families experiencing homelessness, medical vulnerability, and systemic barriers, guiding them from the street into stable homes.

Our services include case management, medical coordination, behavioral health and substance use counseling, and housing navigation. We operate recuperative care centers, emergency and supportive housing facilities, and family-focused programs, including micro-communities. We offer programs like integrated healthcare, financial literacy, licensed therapy, in-home counseling, tutoring (including Private Professor and Mini Med School), child care, and transportation to medical appointments.

In fiscal year 2025, we served 2,329 medically vulnerable individuals in recuperative care, housed 1,719 people, and provided 450,886 safe bed nights. We also opened the nation’s first Children & Families Recuperative Care Center and are preparing to open the Richard Lehn Intergenerational Campus in 2026. Charity Navigator gives us a 98% rating, noting that 91 cents of every dollar goes directly to our programs. Our financials are independently audited, maintaining a 90% program funding ratio.
