We are a mobile healthcare provider. We combine technology with a full-stack medical practice to deliver care in patients’ homes. We partner with healthcare organizations to help patients get preventive care through convenient in-home visits. These visits are designed to close care gaps and provide actionable data to providers. Our care model combines in-person interactions from in-home “Sprinters” with virtual care from a clinical team.

Our Sprinters are W-2 phlebotomists hired from local communities. We train them in Medical Assistant and Community Health Worker skills. They perform hands-on diagnostics and connect patients with our virtual team of physicians, nurses, pharmacists, and care navigators.

Home visit services include:

  • Blood pressure checks
  • Blood draws
  • Urine collection
  • Colorectal cancer screenings
  • Diabetic retinal exams
  • Diabetic foot exams
  • 12-lead ECGs
  • Pediatric lead screenings
  • Topical fluoride applications

Virtual consultations cover:

  • Family and medical history
  • Routine measurements
  • Medication reviews
  • Personalized prevention plans
  • Benefit education
  • Accurate diagnosis coding

We also offer:

  • Medication support: Adherence support, history retrieval, reconciliation and review, ordering and delivery.
  • Care navigation: In-network visit scheduling and follow-up, primary care provider selection, specialty provider and facility referrals, and social resource referrals.
  • Assessments: Health Risk Assessments (HRA), social needs screenings, race, ethnicity, language, and gender capture, depression screening, fall risk screening, and cognitive function screening.

We achieve a 30% member booking rate, an 80% care gap closure rate, a 90+ patient Net Promoter Score (NPS), and 98% on-time arrivals.
