We were founded in 2020, focusing on virtual cardiac and pulmonary rehabilitation. Our programs help patients recovering from conditions like cardiac and pulmonary disease, COPD, heart attack, heart transplant, and post-stent placement. We deliver evidence-based care at home, especially for those who can’t attend traditional in-person rehabilitation.

Our personalized rehabilitation plans cover exercise, nutrition, and stress management. Patients receive a “Carda Care Package” with necessary devices, including a tablet, shipped directly to their home. Our clinical team of doctors, nurse practitioners, clinical exercise physiologists (CEPs), and exercise coaches provides care. CEPs are the main point of contact for patients, supervising exercise sessions in real-time and monitoring vital signs. Physicians oversee this monitoring. We also use Remote Patient Monitoring, where our CEPs evaluate prescriptions, manage patient caseloads, respond to alerts, and review SpO2 readings from Overnight Pulse Oximetry Monitoring.

Our patient outcomes show significant improvement: 92% feel better after 12 weeks, 98% manage stress better, 82% have more energy, 81% lose a clinically significant amount of weight, and 53% gain aerobic capacity. We accept United Healthcare, Aetna, BlueCross BlueShield, and Humana members, and treat patients from systems like Intermountain, Montefiore, and Beth Israel. In January 2026, we will partner with Humana to expand nationwide access to at-home cardiac rehabilitation. We also contribute to clinical studies with health systems in Pennsylvania and New York. Starting our program involves a free assessment, receiving your Carda Care Package, meeting a physiologist, and then beginning your personalized plan.
