We were founded in 2014 as a physician enablement company, helping seniors get affordable, quality healthcare. Our business model improves medical outcomes and lowers costs by taking a holistic approach to understanding members’ health needs and social risk factors. Our proprietary software, the Clover Assistant, supports this approach by aggregating patient data to give physicians real-time, personalized recommendations during appointments.

We offer affordable Medicare Advantage plans, including PPO and HMO options, with low or $0 premiums and copays. These plans include benefits like dental, vision, and over-the-counter item allowances. Our Clover LiveHealthy Rewards Program lets members earn up to $400 for health-related activities. The plans are designed for affordability and flexibility, with a large network of hospitals and providers that don’t require referrals. We also offer home-based care through our Clover Home Care program.

We manage care for Medicare Advantage members in Alabama, Arizona, Georgia, Mississippi, New Jersey, Pennsylvania, South Carolina, Tennessee, and Texas. Our headquarters are in Nashville, with a distributed workforce across the U.S. and a technology team in Hong Kong. We specialize in Medicare Advantage, complex care, home care, artificial intelligence, machine learning, HEDIS, STARS, and insurance.
