We are a tech-enabled healthcare company that improves care and reduces costs for patients with advanced illness and those nearing end-of-life. We partner with healthcare providers across 25 states to meet patients’ full care needs.

We use claims and product data to generate insights, creating and refining healthcare metrics. Our team uses SQL, dbt, and BI tools like Looker to build algorithms, data models, and clear data visualizations. We also manage the growth of our provider network, handling credentialing and improving workflows.

Our mission is to give patients with advanced illness more good days. We aim to improve care and reduce costs for those with advanced illness and nearing end-of-life, especially seniors. We empower our provider network to focus on the communities they serve.

We are a remote healthcare company, though some roles require travel for on-site company events. Our work environment is a fast-paced startup where we value asynchronous communication and cross-functional collaboration. We have 11-50 employees.
