Patient Success Manager / Health Coach (m/w/d)

Remote-based role - must be in travelling distance to Berlin

Part-time (min. 25 hours per week)

Your Mission

As a Patient Success Manager at HealthHero Germany, you are the face of our program and the primary partner for our patients. You will lead individuals through their digital patient journey—from initial onboarding to long-term health success. By combining clinical expertise with motivational coaching, you ensure that every patient achieves the best possible outcome in our diabetes and obesity management programs.

You will:

  • Serve as a Trusted Health Advisor: Guide patients via video and chat, focusing on medical adherence, behavioural change, and long-term motivation.
  • First-Line-Support: Act as the primary navigator for patients, from technical onboarding on our platform to daily support with medication handling (e.g., pen usage and travel planning).
  • Ensure Safety & Triage: Monitor patient progress closely to identify side effects or medical concerns, escalating them proactively to our Medical Team.
  • Own the Community: Foster an environment of support by moderating group chats and building a thriving patient community.
  • Drive Operational Excellence: Document patient progress, facilitate timely delivery of prescriptions/medication, and provide the management team with anonymized data on engagement and health outcomes.
  • Bridge the Gap to Product: Act as the voice of the patient, translating direct feedback from daily interactions into actionable insights for our product and tech teams.

Your Profile

  • Clinical Background: You have a completed qualification as a Medical Assistant (MFA), Nurse, or a comparable medical degree.
  • Coaching Expertise: Ideally, you have additional training in Motivational Interviewing, obesity training, or health coaching.
  • Tech-Affinity: You are comfortable using digital tools like Slack, CRM systems, and video conferencing software.
  • Execution & Empathy: You are a strong executor who can manage multiple cases reliably while maintaining a clear, empathetic communication style in German.
  • Mindset: You are entrepreneurial and proactive. You don’t just follow a script; you help us build the “ops playbook” as we grow the team.
  • Languages: You possess excellent German skills (essential for patient contact) and very good English skills for internal team collaboration.
