In this role, you’ll build strong relationships with members, understand their unique behavioral, medical, and social needs, and guide them toward better health outcomes through personalized, compassionate care.
About the IVRP Program
Our Interpersonal Violence Recovery Program (IVRP) provides coordinated, trauma-informed support to individuals who have experienced interpersonal violence. Through highly personalized interventions, you’ll help address safety concerns, social determinants of health, and barriers to care—empowering members on their path to recovery while reducing avoidable healthcare costs.
What You’ll Do
Connect with individuals through meaningful outreach, education, and enrollment support
Partner with members to develop personalized Service Plans based on their needs, goals, and care gaps
Help members build practical skills and strategies to manage challenges and reduce hospital visits
Coordinate community resources to support behavioral, medical, and social needs
Serve as an advocate and liaison between members and their care teams (family, providers, and community partners)
Support members with scheduling, follow-ups, and accountability
Identify and escalate any critical incidents or quality-of-care concerns
Track outcomes, productivity, and program impact
Maintain accurate, timely documentation and records
What You’ll Bring
Bachelor’s degree in nursing, social work, or a related health/human services field
Experience in case management, behavioral health, or healthcare (preferred)
Certifications such as CCM, CHW, PSS, or ACM (a plus)
Strong knowledge of community resources, social determinants of health, and behavioral health challenges
A compassionate, proactive approach to supporting individuals facing complex life circumstances
What We Offer
Compensation: $20.67–$23.08/hour ($43,000–$48,000 annually), based on experience and education
