Pair Team

We founded Pair Team in 2019 as a public benefit company to reimagine Medicaid and Medicare as a connected care ecosystem. We focus on closing gaps, bridging problems, and connecting people with proactive, personal, and whole-person care. We help individuals overcome medical, behavioral health, and social barriers through coordinated care, blending empathy, AI, and local partnerships. We connect vulnerable patients to local care teams that help them access physical, mental, and social services, including support for housing, food, transportation, scheduling primary care providers, and speaking to behavioral health specialists. We partner with over 100 local organizations, clinics, and hospitals to provide support. Our community-integrated model has led to increased outpatient care, fewer emergency department visits and hospitalizations, and measurable health improvements for Medicaid patients, according to a peer-reviewed study published in the Journal of General Internal Medicine. We operate in the Hospitals and Health Care industry, specializing in Care Coordination.
