Our web-based platform gives providers and their staff real-time recommendations and payments for value-based actions at the point of care.

For payors, we integrate quality, risk adjustment, and total cost of care activities into provider workflows. We aim to transform value-based care with monthly payments and transparent systems. For providers, our platform turns actions like scheduling follow-ups or documenting chronic conditions into predictable monthly revenue through reimbursements.

Our platform uses comprehensive claims data and other information to address patient needs. It has dynamic incentives that adjust in real-time to optimize performance, an easy-to-use interface with strong EHR integration, and real-time reporting for insights and tracking.

We partner with payors and providers in over 37 states, managing more than 1.2 million lives on our platform. Over 20,000 providers and 2,200 medical groups use our system. Results include an 11% increase in Annual Wellness Visit completion, a 7% increase in accurate HCC coding capture, and a 0.5 point increase in Quality STARS.
