We provide a value-based care platform and an Accountable Care Organization (ACO) solution for independent primary care physicians who participate in Medicare’s value-based care program. We help practices build stronger patient relationships with personalized care. We offer value-based incentives, technology, and a virtual clinic to help practices succeed in these arrangements. Our care model helps over 350,000 Medicare beneficiaries across 30 states manage chronic conditions with their doctors.

We founded Vytalize Health in 2014, starting with a practice in New York City that offered whole-person care, including ²⁴⁄₇ support and virtual or in-home patient visits. By 2016, we expanded to New Jersey. In 2018, we created a partnership platform to bring Medicare-focused care to practices and share cost savings.

We also pay doctors for spending more time with patients, focusing on preventive care, participating in weekly calls, and engaging with patient outcomes and staff performance. Our platform supports private practices, group practices, community health centers, and existing ACOs.

We offer several benefits, including a fully remote and flexible work environment. Employees can earn an annual bonus. We provide competitive compensation and a 401K match, contributing 100% of up to 4% of an employee’s annual salary. Health benefits start on your first day, with 100% coverage for a base plan and up to 90% coverage on all other plans for individuals and families. We also provide a quarterly health and wellness stipend and a home office investment stipend to help employees set up their workspace.
