About CareBridge

We provide value-based care for individuals receiving home and community-based services (HCBS). We partner with managed Medicaid and Medicare plans to help members maintain their health, independence, and quality of life. Since our founding in 2019, we have served over 100,000 members. Our solutions have helped increase independence, improve clinical quality, and reduce emergency room visits, hospitalizations, and overall costs. We’ve also achieved a Net Promoter Score of 84.

We offer several solutions:

  • 247 Member Support: This service gives members and caregivers a cellular-enabled tablet for round-the-clock access to our medical team, which includes physicians, psychiatrists, pharmacists, nurses, and social workers.
  • Decision Support: We provide recommendations for HCBS, based on person-centered assessments by LTSS Specialists (who have physical or occupational therapy backgrounds) and data from our large HCBS analytical database.
  • Electronic Visit Verification (EVV): We use EVV technology to confirm services received and home healthcare visits.
  • Data Aggregation: We also offer data aggregation services.

We are based in Nashville, TN, and have a national presence, with both local and remote team members. We offer remote and in-office work. We recently joined Elevance Health.
