Clarify is seeking a motivated and experienced Physician Liaison to engage directly with healthcare providers and practices to support Clarify’s outcomes-based partnerships. In this role, you will guide providers/practices through sustained behavior change, including optimizing utilization of in-network, high-quality, and efficient care delivery. You will combine data-driven insights and structured change management approaches to build provider commitment and deliver measurable partnership outcomes.

The Role
As a Physician Liaison, you will serve as a trusted advisor to healthcare providers and practices, guiding them through a structured change journey to optimize utilization of in-network, high-quality care delivery. You’ll leverage Clarify’s platform and engagement playbooks to diagnose each provider or practice’s readiness for change.
This role goes beyond traditional provider relations. You will function as a change agent, combining consultative relationship skills with a systematic, data-informed approach to influence providers/practices to achieve sustained behavior change.
What you’ll do
Provider Partnership & Relationship Management
Build and maintain trusted, collaborative relationships with assigned providers (physicians, practice administrators, and staff)
Conduct regular interactions (both virtual and in-person) to review performance, co-develop improvement plans, and address barriers to change
Serve as the primary point of contact and advocate for assigned providers, representing their needs and perspectives internally
Education & Enablement
Engage with practice administration, staff, and providers to support workflow improvement, education, and awareness on value-based care program requirements, quality measures, and performance incentives
Collaborate with internal teams to resolve barriers that may inhibit provider adoption
Performance Analysis & Insights
Translate complex data into clear, actionable narratives tailored to each provider and/or practice
Monitor progress on initiatives, track leading indicators of behavior change, and adjust strategies based on results
Documentation & Engagement Tracking
Maintain detailed records of interactions, change stage assessments, and engagement activities in CRM (Salesforce)
Provide regular reporting on territory performance, behavior change trends, and forecasted impact
What we are looking for
Bachelor’s degree in healthcare administration, Nursing, Public Health, Business, or related field
2–4 years of healthcare experience in one or more of the following areas:
Provider engagement, network relations, or account management
Value-based care program operations or implementation
Healthcare consulting or practice transformation
Proven ability to establish trust and credibility through partnership-oriented, non-transactional engagement
Proficiency in interpreting healthcare utilization data, cost trends, referral patterns, and quality metrics; comfortable working with analytics platforms and Excel
Excellent verbal and written communication with the ability to translate complex data into compelling, provider-specific narratives; comfortable presenting to both clinical and administrative audiences
Ability to independently manage a territory, prioritize activities, and drive results with minimal supervision
Proficient with CRM systems, analytics platforms, and structured engagement tools
Willingness to leverage AI-powered tools and insights to enhance engagement preparation, analysis, and strategy development
Consultative mindset with genuine desire to help providers succeed
Comfort with ambiguity and the ability to read interpersonal dynamics, adapting your approach in real time
Strong work ethic and accountability for territory performance and behavior change outcomes
Team player who shares knowledge, effective tactics, and best practices with colleagues
Nice-to-have:
Master’s degree (MHA, MBA, MPH, MSN)
Practice operations experience
Exposure to change management methodologies or behavioral science concepts
Experience with ACO operations, Medicare Shared Savings Program, or commercial value-based care contracts
Physician Liaisons will ideally be located within the Houston, TX area or assigned territory/region, as in-person visits are an imperative component of our work. Will consider candidates outside of the region who are willing to travel.

$90,000 - $105,000 a year
