Overview
Chronic Care Staffing is seeking a compassionate and experienced Remote Care Coordinator (RN) to join our growing team. This role is ideal for a nurse who thrives in a structured, remote environment and is passionate about improving outcomes for patients managing chronic conditions.
As a Care Coordinator, you will support patients through Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM) services. You will play a key role in strengthening communication between patients and providers while ensuring high-quality, compliant care delivery.
Responsibilities
Conduct monthly Chronic Care Management (CCM) calls and complete verbal enrollments
Perform Health Risk Assessments (HRAs), Transitional Care Management, and Remote Patient Monitoring services
Educate patients and families on chronic conditions, medications, and treatment goals
Collaborate with providers and internal teams to ensure effective care coordination
Encourage adherence to care plans and promote patient self-management
Accurately document all communications and interventions in EHR systems
Maintain HIPAA, CMS, and company compliance standards
Meet productivity and call volume expectations while delivering empathetic, patient-centered care
Active, unencumbered RN license
Must live in MST or PST (WA, OR, CA, ID, NV, AZ, UT, CO, WY, MT, NM)
BLS certification
High proficiency in EHR systems
Recent experience in a clinical or CCM setting
Proficiency in Google Suite
Strong problem-solving, clinical reasoning, and critical thinking skills
Excellent verbal and written communication abilities
Ability to work independently in a HIPAA-compliant home office (locked door, distraction-free environment, two monitors, high-speed internet)
Nice to Have
- Prior experience in care coordination, chronic care management, transitional care, or remote patient monitoring
- Familiarity with community resource navigation to support patient care
- Experience in patient and family education on chronic conditions and medications
- Background in quality measures, CMS compliance, or reporting
- Demonstrated team-player mindset in a remote environment
- Bilingual in English and Spanish
