Founded in 2015, we are headquartered in Boston, with offices in Singapore and India. Our AI-driven solution collects and analyzes over 120 biomarkers in real time, detecting shifts that require early intervention. This allows us to provide hospital-level services, virtual provider networks for remote care, and clinical trials for patients at home. We merged with CopilotIQ to create an end-to-end platform for AI-driven in-home care.

Our platform tackles challenges in the health and pharmaceutical industries, from drug development to care delivery. We help patients transition across different levels of care, clinical trials, and drug companions, providing continuous care and more options. We support patients from pre-surgical optimization through chronic care. Our results include a 70% reduction in 30-day readmissions, the ability to detect deterioration 21 hours sooner, and up to a 38% reduction in care costs.

We partner with risk-bearing organizations, life science companies, over 50 global health systems, and payers to expand access to care and treatments. Our services include Hospital at Home, Timely Discharge, SNF at Home, and Remote Patient Management. We are licensed to operate in 25 U.S. states.
