Your Role
As a Pharmacist with Evergreen Nephrology, you are responsible for ensuring the best clinical care for your patients. You will work collaboratively with an assigned panel of physicians and care managers to manage the medication regimen for patients living with kidney disease and other chronic, complex diseases. This includes the opportunity to work with many different members of the interdisciplinary team including registered nurses, nurse care managers, health coaches, dieticians, pharmacists, psychiatrists, social workers, and even cardiologists and podiatrists – all within the Evergreen Nephrology family.
Role Responsibilities
Some responsibilities may vary based on specific patient programs, but this role’s primary duties include the following:
Establish pharmaceutical therapies that promote safe and effective use
Consult on medication therapy management including medication reconciliation, for optimal drug product selection, dosage regimen design, and effective administration and drug therapy monitoring services
Seek alternatives as necessary to ensure the best care is provided that will maximize clinical outcomes and support appropriate levels of financial spend
Manage all aspects of medication therapy, with a diligent focus on effective medication therapy; identify unnecessary drug therapies, incomplete immunization records, suboptimal therapies, and adverse reactions and monitor for significant drug contraindications
Manage all phases of therapeutic medication plans for effective, safe, and cost-effective medication treatments
Analyze and recommend cost-savings strategies based on plan formularies, insurance phases, and evidence-based guidelines
Present responses to requests for information on specific medications and their therapies
Act as an internal subject matter expert, developing and delivering education to patients and/or caregivers regarding medications and chronic disease states
Drive quality measures and appropriate quality metrics
Other duties consistent with this role, as assigned.
Required Qualifications
Doctorate or bachelor’s degree in pharmacy
You hold an unrestricted Pharmacy state license in TN, AL or CA or will acquire one within 60 days from the time of hire
You have a strong comprehension of pharmaceutical therapies and are focused on quality of life for your patients
You are comfortable delivering educational seminars/instruction on pharmaceutical-related topics to clinicians and patients
Intermediate skills with MS Office Suite of products including Outlook and Teams
Able to work effectively in a primarily remote environment:
- Home internet must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps. Cable, Fiber, or DSL connections hardwired to the internet device are recommended
- Evergreen will provide remote employees with telephony applications and equipment to meet the business requirements for their role
- Employees must work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
Preferred Qualifications
- PGY1 residency or Board Certification
- Multi-state licensure
Compensation
The pay range for this role is $145,000-$155,000 annually. Exact pay is determined based on experience, education, demand for the role, and other role-specific factors.
This role is also eligible for a quarterly bonus.
