We provide value-based kidney care, partnering with healthcare payers and providers. We use technology, including AI, care interventions, and local provider integration, to support patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD). We collaborate with commercial and Medicare Advantage payers, Medicare, health systems, and physicians through flexible value-based payment arrangements, including risk-based programs. We serve over 145,000 individuals with CKD and ESKD across 50 states and partner with over 6,500 providers.

Our technology platform, CareMultiplier™, is HITRUST-certified, and our case management and population health programs are accredited by the National Committee for Quality Assurance (NCQA). Founded in 2018, we operate within the Hospitals and Health Care industry.

Our approach uses a high-touch, coordinated care model with predictive data. This model includes interdisciplinary clinical teams and community health workers. We focus on risk adjustment, HEDIS initiatives, and healthcare economics to evaluate financial performance and find savings.
