Founded in 2017 and launched in the U.S. in 2023, we offer an integrated specialty care platform for complex cardiopulmonary patients. We help prevent and manage flare-ups and provide continuous care. We partner with health plans, risk-bearing groups, and specialists like cardiologists and pulmonologists.

We offer personalized care for conditions such as COPD, asthma, heart failure, emphysema, cystic fibrosis, neuromuscular disorders, hypoxia, insomnia, narcolepsy, restless leg syndrome, sleep apnea, hypertension, high blood pressure, stroke, atrial fibrillation (Afib), and arrhythmia.

We provide tailored services for different groups:

• For patients:

  • Care plans developed with physicians and care coordinators
  • FDA-cleared at-home monitoring devices
  • On-demand virtual or in-person visits
  • ²⁴⁄₇ data-driven support

• For healthcare providers:

  • Remote patient monitoring tools
  • Help reducing no-shows
  • Data-driven decisions through an integrated dashboard that combines outreach, diagnostics, and analytics

• For health plans:

  • A virtual-first care model to diagnose, monitor, and manage members proactively
  • Goals include reducing hospitalizations, lowering costs, and improving outcomes

Our services are covered by over 100 health plans, including Anthem-Emblem, Centene, Medicaid, Humana, Cigna, Medicare, and UnitedHealthcare. We are headquartered in Boston, MA, with an office in Stockholm, Sweden. We offer virtual consultations, and in-person consultations will be available in Tucson, AZ starting February 2025.
