Our mobile app gives patients tools, access to their care team, and customized resources. It supports remote patient monitoring (RPM) and tracks health metrics like protein intake, activity, stress, and sleep. We refine care plans in real-time based on patient needs and symptoms, working with patients and their referring physicians. We integrate detailed adherence data directly into provider EHRs monthly. This allows for early problem detection and care plan adjustments without disrupting existing workflows.
