We launched TailorCare in 2023 to improve patient outcomes for joint, back, and muscle conditions. We use a personalized, data-driven approach to help patients choose and navigate effective treatment pathways. Our process combines a patient’s clinical assessment, health history, preferences, and goals with predictive data and evidence-based guidelines.
Our care navigation services include clinical assessments, patient education, provider matching, and ongoing communication. We connect patients with specialists, coordinate appointments, explain treatment options, and keep their entire provider team updated on progress.
Our licensed physical therapists, who we call Clinical Navigators, conduct evidence-based assessments covering symptoms, health history, lifestyle, and preferences. They then discuss recommended treatment options with patients to ensure they understand and agree.
We build preferred relationships with orthopedic surgeons, physical therapists, and occupational therapists. Our recommendations are based on a provider’s track record of successful outcomes, reputation for quality, and specialized fit for each patient.
We serve patients, primary care providers, payers (including health plans), and specialty care providers (orthopedic, physical therapy, and occupational therapy groups). We function as a complete musculoskeletal solution, assuming full risk on musculoskeletal spending and offering our payer partners shared or guaranteed upside.
