About Wider Circle
We founded Wider Circle in 2015 as a neighborhood-based health organization. We partner with health plans and physician groups across the country to deliver community care programs. Our technology platform connects health plan members with neighbors so they can inform, support, and motivate each other about their health.
Our Connect for Life® program offers both in-person and virtual activities. These programs are tailored for members with similar interests and life experiences, helping to reduce hospitalizations, improve health outcomes, and lessen social isolation. We operate in over 320 communities and offer programs in 5 languages.
During the COVID-19 pandemic, we offered specialized initiatives for vulnerable populations, including virtual programming and peer-to-peer community support. In 2025, Inc. 5000 recognized us as one of America’s fastest-growing private companies.
We also offer a program called “Hank” that organizes local and virtual events for the 55+ community. Our services help members schedule doctor’s appointments, connect to food resources, provide health education, conduct health risk assessments, and link them with social determinants of health resources.
