Overview
Chronic Care Staffing (CCS) is seeking a compassionate and detail-oriented Remote Care Coordinator (CMA/RMA) to join our growing healthcare team. This role is ideal for a certified medical professional who thrives in a remote environment and is passionate about supporting patients managing chronic conditions.
As a Remote Care Coordinator, you will serve as a trusted point of contact for patients, providing consistent outreach, education, care coordination, and documentation — all from a HIPAA-compliant home office.
Responsibilities
As a Remote Care Coordinator, you will:
- Conduct monthly Chronic Care Management (CCM) calls and verbal enrollments
- Perform Health Risk Assessments, Transitional Care Management, and Remote Patient Monitoring services
- Communicate with patients and families about chronic conditions, medications, and treatment plans
- Collaborate with providers and healthcare staff to ensure coordinated care
- Promote adherence to individualized care plans and health goals
- Support medication reconciliation and patient education
- Connect patients with community resources to enhance outcomes and satisfaction
- Maintain compliance with CMS guidelines and organizational regulations
- Meet expected call volume goals and document thoroughly in EHR systems
- Uphold CCS’s reputation for compassionate, patient-focused care
To be considered, candidates must have:
Must live in either MST or PST time zone
Unencumbered active CMA or RMA certification
Active BLS certification
High proficiency with EHR systems
Recent experience in a clinical or CCM setting
Proficiency in Google Suite
Ability to work independently in a HIPAA-compliant home office, including:
- Locked door
- Distraction-free workspace
- Two monitors
- High-speed internet
Strong clinical reasoning, critical thinking, and problem-solving skills
Excellent verbal and written communication abilities
Preferred Qualifications (Nice-to-Have)
Strong candidates may also have:
- Prior experience in care coordination, chronic care management, transitional care, or remote patient monitoring
- Familiarity with community resource navigation to support patient care
- Experience educating patients/families on chronic conditions and medications
- Background in quality measures, reporting, or CMS compliance
- Demonstrated team-player mindset in a remote environment
- Additional coursework or certification related to care or case management
