We redesign healthcare for rural Americans. We connect payers, providers, and community organizations through partnerships. We take full financial accountability for clinical outcomes and the total cost of care across rural counties, which helps align incentives and strengthen local healthcare. Our Homeward Navigation™ platform uses data to connect members to care and local resources that address social determinants of health and improve overall health. We employ care teams that work with local practices to reach people who cannot access care otherwise.
We partner with health plans, local providers, and community groups to offer accessible, technology-enabled care. For members, we provide local care teams who work with their doctors and help them navigate insurance benefits. For providers, we extend clinical support to their patients, helping to improve outcomes. For health plans, we offer a rural care delivery model that improves member experience and builds a strong healthcare ecosystem. We are also developing an integrated machine learning, data, and infrastructure platform with AI tools to support our care delivery.
