We started in 2020, partnering with primary care providers and healthcare organizations to deliver value-based care. We provide technology, services, and financial tools to help them give proactive patient care, cut healthcare costs, and perform better in Traditional Medicare and Medicare Advantage models.

Our AI-powered platform offers predictive insights for high-impact outreach, automates workflows, and integrates with existing systems to support care teams. We also offer financial incentives like upfront payments, monthly payments, and tiered incentives to align financial goals and stabilize cash flow for providers. We use our own data science and advanced modeling to assess provider readiness and match participants to suitable payment models.

We have expanded to partner with thousands of primary care providers in practices and organizations across 44 states. Our investors include Andreessen Horowitz, Viking Global Investors, AlleyCorp, and SV Angel.

Our benefits include flexible PTO (with a two-week minimum) and 15 paid holidays. We provide comprehensive health coverage through Blue Cross Blue Shield PPO (our base plan is fully funded), along with dental, vision, FSA, commuter, disability, life, and wellness programs. We offer a 401K match of 100% up to 4.5%, funded every pay period. Parental leave includes 16 weeks paid for birthing/primary caregivers and 12 weeks for partners. Employees also receive up to $200 per quarter for home office reimbursement (for internet, phone, or office supplies).
