Founded in 2017, we developed a web-based digital platform to help transfer patients from hospitals to aftercare providers. Our platform includes a smart search tool for sending targeted requests and enables real-time, secure communication between hospitals and care providers. This makes the process faster and more transparent for hospital staff, care providers, and patients.

Our solutions simplify discharge and transfer coordination between hospitals, care facilities, rehab clinics, and homecare and medical aid companies. For hospitals, our platform and AI tools streamline discharge management, helping optimize patient length of stay. We are also developing an AI toolkit to automate administrative documents. Care facilities use our platform to manage occupancy by coordinating hospital requests or actively searching for patients. Our AI also supports their admission management. Rehab, homecare, and medical aid companies receive care requests from hospitals and transfer documents digitally through us.

Over 1,000 acute hospitals use our platform for discharge management, and our network includes more than 26,000 aftercare providers. We have helped 750,000 patients. We are ISO 27001-certified, comply with DSGVO (GDPR) regulations, and process patient data exclusively within the European Union. We have received funding from the European Regional Development Fund and Investitionsbank Berlin.
