Location: Remote, with one required in-person training in New York City

Type: Summer Internship

Compensation: $20/hour, full-time (40 hours/week)

About the Opportunity

We’re looking for a Patient Navigator Intern to support our Health-Related Social Needs (HRSN) care navigation program. Working alongside clinical and operations teams, you’ll help patients overcome real-world barriers - housing instability, food insecurity, transportation gaps - that stand between them and consistent, quality care. This role functions similarly to a case manager and is an ideal fit for someone pursuing a career in community mental health, social work, public health, or healthcare operations.

What You’ll Do

  • Screen patients for health-related social needs: Conduct structured assessments using established screening tools and internal workflows.

  • Connect patients to community resources: Help individuals navigate housing programs, food access, transportation support, and other essential services.

  • Provide ongoing navigation support: Follow up with patients to ensure continuity and address emerging barriers as they arise.

  • Collaborate across teams: Partner with clinical and operations staff to support individual care plans and document progress accurately.

  • Support community outreach: Participate in engagement efforts to reach patients who may not yet be connected to services.

What You’ll Learn

  • Care coordination: Hands-on case management experience with Medicaid populations.

  • Social determinants of health: How unmet social needs directly shape clinical outcomes.

  • NYC public systems: Familiarity with community resources, public programs, and local support networks.

  • Behavioral health operations: Working within an integrated care model using remote tools and documentation workflows.

Who we’re looking for

A current undergraduate or graduate student, or recent graduate, in social work, psychology, public health, healthcare administration, community health, or a related field. You bring:

  • Genuine interest in community mental health, care navigation, or healthcare support

  • Comfort discussing sensitive topics with empathy and professionalism

  • Strong communication, interpersonal, and organizational skills

  • Ability to work independently in a remote environment and stay accountable

  • Openness to feedback, supervision, and mentorship

  • Willingness to learn new systems and remote workflows quickly

Internship Structure

  • Fully remote internship

  • One required in-person training in New York City (approximately 3 hours)

  • Full-time, 40 hours per week for the duration of the summer internship

  • Training provided on screening tools, navigation workflows, documentation processes, and remote systems

  • Supervision and mentorship from experienced team members
